What to bring to your appointment:
- Referral
- List of current medications
- Medicare card
- Private Health Insurance details including membership number
- Healthcare card or DVA card
- Imaging: cds, hard copies, scans, ultrasounds, MRIS, X-rays, CT scans
- Copies of the radiologist’s report if you have them
Please contact us at least 24 hours before your appointment so we can contact the relevant imaging department to obtain online access, if you do not have a copy.
